Liability Release & Parental Agreement
Please note, the parent/guardian should print this agreement, sign, and return it to
the Center prior to student enrollment.
LIABILITY RELEASE & PARENTAL AGREEMENT

This release has been given in consideration of the acceptance by the
Center for Peace, to participate in the program. The terms and
conditions stated in the email are inherent, binding and agreed to by
me/my guardian/parent(s)/family. Applicants, whether or not a minor,
guardian or parent signing this release understands that this is a
release of liability and parental agreement contract.
I/We have carefully read and fully understand its contents.
I/We have been fairly and honestly informed that risks and dangers
are inherent in program activities and that reasonable care, caution,
instruction, supervision and expertise cannot eliminate hazardous
exposure.
I/We further understand that injuries, minor and severe, may occur
during the activities.
I, for myself, my son/daughter/ward and heirs, release The Center
for Peace, its agents, officers, employees, directors, guide
services named or not named, of any and all legal liability for any
injury or death or loss/damage to property caused by or resulting
from my or my child's/ward's participation in this trip, regardless
of the cause or causes.
I/We further understand that medical insurance is required and is
the responsibility of each participant to cover any and all medical
requirements and expenses.
I/We waive any claim caused by or resulting from any failure to
follow instructions.
I/We have read, understood and agree with all terms and agreements
set forth in the Participation/Unity Agreement. No representation
or warranty of any kind has been made other than those that are
contained in the Center for Peace package.
I/We hereby grant permission for the child to participate in all
activities organized by or through the Center for Peace. We grant
permission to make photographic records for promotional purposes
without recourse or compensation. We give permission to cross
international borders and use public or private transport.
I/We understand and agree that this Application -- including the
Liability Release -- will be interpreted under the laws of the
State of Maryland.
I/We further agree that the courts of Maryland shall have exclusive
jurisdiction to settle any dispute and/or controversy of whatsoever
nature arising out of or relating to this Application including the
Liability Release -- or my/my child's participation in any activity
organized or sponsored by the Center for Peace, and that accordingly,
any suit, action or proceeding arising out of or relating to any such
matter shall be brought in such courts.
I/We agree and understand that the Center for Peace contracts with
the foregoing Liability Release covers all partners involved in the
organization and implementation of the Center's projects.
I understand that extreme serious offense such as sexual harassment and
fighting during the camp may lead to my son/daughter being placed on the
first flight home at the parents' expense.
On the first offense (non extremely serious), a verbal warning will be
given to the student.
On the second offense (non extremely serious), parents may be contacted
via phone or other means to discuss the situation. A Letter of apology
may also be written to the Center and the offended parties.
The third offense will be treated as an extremely serious offense and
treated as such.
In case damages occur as a result of non related Camp sanctioned
activities, parents/guardian may be required to pay for the damage done
by their dependent, son or daughter.
